Job Description:
Job DescriptionHome Health Advantage is growing, and we are
recruiting for a dynamic RN to visit patients in their home and
coordinate care for patient and family. You are an integral part of
the patient's success in managing at home as you will be
coordinating with referral sources (hospitals, clinics, senior
facilities) and physicians. Must have outstanding clinical
assessment skills, great communication skills, ability to teach
patients about their medications and disease process so patients
can learn self-management. Patient caseload would be traveling from
Aurora/Naperville south on 355 to Joliet/ Lockport/Channahon. You
will need to be a teacher and patient advocate so patient goals of
care are exceeded! Our mission is to exceed patient/family
expectations and focusing on care excellence with every
interaction!!Requirements: RN IL License, CPR, recent TB testing
within 6 months, reliable transportation. Ability to work
independently.Geographic Area: Naperville, Bolingbrook, Oswego,
Bolingbrook, Joliet, Plainfield, Shorewood, Joliet, Crest Hill,
