This is an exciting new opportunity to join Charter Senior Living as a Regional Director of Health and Wellness.-- The position is responsible for directing the health and wellness of our residents and ensuring the quality of care is being provided at each assigned community.-- The person will be providing clinical support and expertise to Regional Director of Operations, Executive Directors and Wellness team.-- The position will conduct scheduled site visits to ensure compliance with Charter standards, state and federal regulations.-- Must be able to travel.

Qualification's:


  • Must be a Licensed Nurse with Associate or Bachelor's Degree
  • Maintains a current state license per state regulations
  • Has Familiarity working with the elderly.
  • A minimum of four years in clinical nursing practice required or long-term care administration or supervision required.
  • A minimum of two years of management experience.
  • Demonstrated ability to identify problems in a timely manner, develop effective resolutions to those problems and ensure implementation of the proposed resolution, confirm results and revise as necessary.
  • Multisite experience preferred but not required.

    Essential Duties

    • Conduct audits of Care plans to ensure completion and accuracy.
    • Implement policies and procedures for Health and wellness.
    • Participate in on-boarding process for new Health and Wellness Directors.
    • Review and investigates resident/family complaints as directed by the VP of Health and Wellness and/or Regional Director of Operations.
    • Reviews all potential move-outs due to level of care concerns prior to any notifications.
    • Steps into open Health and Wellness Director positions during transition as needed.
    • Schedule and complete site visits to communities and ensure follow up with Health and Wellness Directors while providing guidance, clinical expertise, communication, and consistent leadership training.
    • Audit files to ensure smoking, fall risk, safety evaluations, nursing assessments are completed in conjunction with service plan meetings.
    • Audit files to ensure the service plan dates are according to state regulation guidelines.
    • Become familiar with the service plan program and all of the reports it can create.
    • Audit files to ensure home health, hospice and ancillary care providers, communicate their visits and provide necessary documentation of their visit.
    • Audit incident reports and ensure the incident investigations are completed with appropriate interventions and all notifications have been made.
    • Audit files to ensure that all health services staff have the necessary training required for their job. (CPR, First Aid, and dementia training, if applicable).
    • Review policies and procedures for accuracy.-- Recommend to the home office any changes and/or updates that are required.
    • Conduct site visits as per the survey schedule provided by the home office, or as needed.
    • Perform such other tasks as may be required from time to time by the management of the property.
