RN Navigator, SpineGENERAL SUMMARY:# Functions within a
multidisciplinary team of physicians, office operations and acute
care services team members to ensure that patients receiving spine
care receive quality and comprehensive services throughout the care
continuum. Serves as the clinical resource with expertise in spine
care management. Collaborates with physicians, healthcare
professionals, and administration to identify opportunities to
improve patient experience and care delivery. Analyzes outcomes
data and reviews current literature to recommend process
improvement initiatives. Serves as liaison across the healthcare
system and community regarding services provided for this patient
population. Promotes collaboration across ambulatory and acute care
spine program stakeholders. Develops and delivers patient education
and serves as a resource for patients and their care partners. #
HOURS: Full-time, M-F, 8:00am-5:00pm # PRINCIPAL DUTIES AND
RESPONSIBILITIES: (The following duties and responsibilities are
all essential job functions, as defined by the ADA, except those
that begin with the word #May.#) Demonstrates the knowledge and
skills necessary to provide care to the patient population served.
This includes knowledge of their physical and psychological needs
and the ability to respond appropriately to those needs. Evaluates
surgery-specific teaching materials and processes and identifies
opportunities to enhance education content and expand teaching
modalities. Acts as an adjunct to ambulatory and acute care nurses
to provide education and information to the patient and their care
partner, helping to make care seamless, continuous, and
comprehensive. Reinforces education related to disease process,
surgery preparedness and recovery, pain management, rehabilitation,
and when to notify the physician of postoperative signs and
symptoms. Maintains presence in acute care and ambulatory settings
to observe processes, round on patients, and to serve as a resource
for complex care planning. Acts as a point of contact for spine
patients, care partners, and healthcare providers. Uses nursing
judgement and knowledge of scope of practice to determine questions
and concerns that are to be addressed by RN navigator versus
physician/APP. Establishes rapport and routinely interfaces with
interdisciplinary and community resources to ensure that patients
and providers are connected to comprehensive care management
modalities (rehab, pain management, radiology, etc.). Collaborates
with administration and spine surgeons to determine prioritization
for process improvement and piloting new initiatives based on
outcome data, patient experience, and current research literature.
Initiates self-learning by routinely reviewing literature and
enrolling in appropriate continuing education opportunities
(webinars, conferences, etc.) Actively participates in spine
leadership and staff meetings to provide productive feedback on
program and propose solutions for improvement. Collects and
analyzes data when needed to evaluate program outcomes. #
Education, Experience and/or Skills Required: Bachelor#s degree in
nursing Minimum three years of RN clinical experience Strong
problem-solving and communication skills Ability to function in an
organized and time conscious manner Education, Experience and/or
Skills Preferred: Five years of clinical RN experience Experience
in delivering care to spine patient populations
License/Certification Required: Current State of Illinois
Registered Nurse licensure Current CPR Certification issued by
American Heart Association or American Red Cross
