Job Description:

Social Worker/Discharge Planner Float - Naperville, ILThis position floats to all inpatient units to provides counseling and discharge planning services to patients who in need of ongoing behavioral health and/or medical care following discharge due to changing census and acuity level.-- Coordinating discharge planning at the direction of the patient's treatment team.-- Obtaining appropriate releases, communicating with other care providers and collaborating with patients, families and the treatment team to develop, arrange and document a comprehensive discharge plan.
Required Education, Skills and/or Experience:

Master's Degree in Social Work or a related behavioral health field
Minimum three years of experience in a behavioral health care setting, with a minimum of one year being in acute care

• Ability to maintain information which is highly confidential
  
• Ability to clearly summarize pertinent clinical information via written correspondence and medical records documentation
  
• Ability to demonstrate self-control in volatile or hostile situations
  
• Ability to set therapeutic limits and maintain therapeutic distance with clients; maintains therapeutic boundaries with patients during treatment and following discharge

  Preferred Education, Skills and/or Experience:
  

• 
  A minimum of two years of previous social work experience.

  Required License and/or Certification:
  
  Current State of Illinois licensure with a minimum of three letters which may include LSW (Licensed Social Worker), LCSW (Licensed Clinical Social Worker), LPC (Licensed Professional Counselor), LCPC (Licensed Clinical Professional Counselor).
  
• Current CPR Certification issued by either American Heart Association or Red Cross
