Job Description:

As a committed Crisis Care Coordinator at Lutheran Social Services of Wisconsin and Upper Michigan (LSS), you will play an essential role in providing crucial support and crisis intervention services to individuals and families in distress. By utilizing your expertise in social services, you will coordinate and manage immediate assistance programs, facilitate access to necessary resources, and work collaboratively with various service providers. This position requires strong organizational skills, empathy, and the ability to work effectively under pressure, ensuring that those in need receive timely and effective support aligned with LSS's mission of promoting health, healing, and hope within the community. Assess client needs during crises and coordinate appropriate responses. Collaborate with healthcare providers, social workers, and other agencies to facilitate services. Develop and maintain crisis response protocols and ensure all team members are trained. Provide direct support to individuals and families in crisis situations. Document all interventions and outcomes within client care records. Participate in community outreach programs to educate the public on crisis prevention and response.
