Job Description:

Carle Health is seeking a dedicated Social Worker to join our innovative healthcare team. This role is pivotal in supporting our mission to deliver community-based care, tailored to enhance patient outcomes through direct patient interaction and integrated medical services. The ideal candidate will contribute both in acute care settings and outpatient services, addressing psychosocial components of healthcare and facilitating comprehensive treatment plans. Conduct patient assessments to determine psychosocial needs and appropriate intervention strategies. Collaborate with healthcare professionals to integrate social services with patient care plans. Provide crisis intervention and connect patients with community resources to support their recovery. Advocate for patient access to community resources and services, including housing, employment, and mental health support. Document patient interactions, assessments, and outcomes in accordance with healthcare regulations and Carle Health policies. Strong interpersonal and communication skills Experience in crisis intervention and patient advocacy Ability to navigate complex healthcare systems and improve patient access to community resources Proficiency in maintaining detailed and accurate patient records Capability to work collaboratively in a multidisciplinary team Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Licensed Clinical Social Worker (LCSW) or eligible for certification CPR Certified preferred
