Family Home Health Network, is a provider of intermittent Home Health care, palliative care, hospice care and custodial private care services throughout the Chicagoland area.GREAT FLEXIBILITY. PER DIEM PAY RATES BETWEEN $75-$85!!The Occupational Therapist is responsible for providing occupational therapy services to patients including but will not be limited to; assessing and evaluating therapeutic/rehabilitative/functional status and participating in the development of the total plan of care.
